Capacity Building in the US Virgin Islands
You are asked to participate in a research study on Self Determination and Identity. This form provides you with information about the study. You are asked to complete a questionnaire which will provide demographic and residency as well as questions related to Self Determination and Identity. This will take no more than 10 minutes to complete. Refusal to complete the instrument means the participant has withdrawn from the study. This study is intended for persons who have lived in the Virgin Islands for at least 3 years and are 18 years and older. You can print a copy of this form to keep for your reference. The Principal Investigator or his/her representative will provide you with any additional information that may be needed and answer any questions you may have. Information will be provided on the subject matter if necessary. Your participation is entirely voluntary, and you can refuse to participate or withdraw at any time without penalty or loss of benefits to which you are otherwise entitled. Additionally, your refusal will not influence current or future relationships with The University of the Virgin Islands, the researchers or any others associated with this project. The first 10 participants who fully complete the questionnaire will receive a $25.00 Gift Card to the University of the Virgin Islands Bookstore to assist in compensating for time spent on participation. Throughout the study, the researchers will notify you of new information that may become available and that might affect your decision to remain in the study. If you have any questions, you may ask the Principal Investigator. In addition, if you have any questions about your rights as a research subject, or if you have any complaints, concerns, or questions about the research, you may contact the University of the Virgin Islands Institutional Review Board at (340) 693-1202.
