Instructions For Teacher Or Person Making Trip Request:
1) Requests must be submitted to the Transportation Department TEN WORKING DAYS prior to trip.
2) Below must be completed in full.
3) A separate request form must be filled out for each trip.
This form will forward via e-mail to to your building principal and you will get a copy.
Instructions For Building Principal:
4) Approve by forwarding to the transportation director at
troy.griffith@g.perry.k12.ia.us or reject by contacting the teacher or staff that requested the transportation arrangements.
Instructions For Transportation Director:
5) Approve by forwarding back to the principal with the arrangements, reject by contacting the requestor and the building principal for possible schedule changes.
