Pleasant Valley Junior High and High School will continue with the hybrid learning model beginning on January 19, 2020. ONE HOUSEHOLD MEMBER MUST FILL OUT THIS FORM FOR 7-12 STUDENTS BY 5:00PM ON FRIDAY, DECEMBER 4.

PLEASE SUBMIT THIS FORM ONCE PER HOUSEHOLD. IF YOU'D LIKE TO MAKE A CHANGE BEFORE THE DEADLINE, PLEASE CALL 563-332-5550.
 
***Please note, due to scheduling and staffing, if you select the 100% online learning model, you must stay in the 100% online model through the end of the school year.
 
A survey will follow in January to allow elementary families to choose a learning model for the third term starting February 16.
