2025 Affiliate Player Tracking Form
This form is to be filled out as the official Affiliate Tracking Form for the 2025 season. 

Please read and review all rules & regulations in our Virtual Handbook here regarding Affiliate Players. 

General Info:
  • Only players registered under Baseball Alberta (confirmed member in Spordle under their Home Association + added to the team roster under "affiliate players") are eligible for teams to use as affiliate players.
  • No Affiliate players will be permitted for play at Provincial Championships.
  • No affiliates will be allowed for the Provincial ‘A’ division.
  • The team MUST phone local coach to determine player availability, then phone parent for permission (unless player is 18 years of age)
  • Affiliate players may only sign with 1 team and may only play up to 5 games throughout the entire schedule.
*You will only have to send one single payment of $25 per player used, NOT per game. The player will be considered approved by Baseball Alberta if the following form has been submitted, and we have recieved the single fee for the player.

If you choose "No", the fee can be sent to etransfer@baseballalberta.com. Please leave in the description (Affiliate Fee - Player Name) or the payment will not be recorded. 

If you have chosen "Yes", no payment is required if an E-Transfer has already been sent.

No refunds will be processed after the E-Transfer has been accepted.
