Families who wish to switch their student’s device from a waiver device to a school-issued device may do so by filling out this form.

Students who transition should expect to receive a used or older model device that aligns with the district’s transition schedule and model of device deployed to the other students in that grade level.
        Ex. A student who elects to use a waiver device for 9th, 10th, and 11th grade, but chooses a district selected device in 12th grade should expect to pay $40 for that year and receive a 3-year-old device similar to the device his or her classmates use.

Please remember that students with a school-issued device are assigned a device fee of $40 per year with 3 repairs per year. This accumulates to $160 over 4 years after which the student keeps the device. Grades K through 4 differ. Grades 2 through 4 receive the same 3 repairs per year, but do not keep the device upon exit of 4th-grade and have a $20 fee. The device fee is waived for students who qualify for free or reduced lunch.
