2025-2026 Medfield Afterschool Program Jump Start Registration
Jump Start MAP is an enriching option for children who are 4 & 5 years old and not yet in Kindergarten. Children participating in the Memorial School Integrated Preschool, can be picked up or dropped off from MAP on their scheduled days.  The Jump Start Program does not operate on early release days, non school days, or over the February & April Vacation weeks.  MAP must have a minimum number of children enrolled in order for the program to run each day. Please complete one form per child.  

Registration Deadline: Friday, March 7, 2025.  Enrollment at MAP is subject to space availability. Current MAP families have enrollment priority. Late registration, while accepted, are also subject to space availability.    Please reach out to the Executive Director, Annette, if you have any questions: annette.map@comcast.net.  Thank you!

To complete the registration process, please mail MAP a $50.00 registration fee ($30.00 for each additional sibling) by the deadline:  Friday, March 7, 2025. MAP holds the registration fee until we are able to offer you enrollment in the program.  Registrations are incomplete until MAP receives your registration fee check.   NEW Families must mail a check to MAP, P.O. Box 18, Medfield, MA 02052 postmarked by the deadline. Please write your child's name and grade in the memo section of the check. Current MAP families will be billed their registration fee with their 3/14 tuition billing statement.   Please type your name below as your signature.  Thank  you! *
