Please note places will be allocated on a first come, first serve basis but priority will be given to those in receipt of Pupil Premium and/or those who have not previously attended a club. Completing this form does not guarantee your child a place. You will receive a Parent Mail to confirm your child's place when the form closes / the club reaches capacity.

Collection arrangements: all children attending an afterschool club must be collected by an adult from the school playground.
