Please fill out this form if you have any concerns about how your first name might be displayed in the yearbook. That includes if you have a preferred name that isn't on your official TDSB/legal records, or if your records have an incorrect version of your name.

Note that we are taking SERIOUS REQUESTS ONLY. There is zero tolerance for unserious requests. 

If you don't submit this form, your name will appear as it does on your TDSB records.

Also ensure to check for typos! We'll write your name as it's submitted with the first letter capitalized, but ensure that you capitalize it properly anyway. If there are additional capitals included, they will be assumed to be intentional. There's also a notes question at the end if you need to clarify anything regarding spelling.

The deadline for this form will be the end of the day on December 14th and we won't accept responses after. Please make sure you fill this form in on time, and spread the word to your friends in case one of them wants to do it too!
