Faith Formation Registration preK-8th 2026-2027
Welcome! Please use this form to register children for St. Francis of Assisi Faith Formation programs. 

Please note preK (Atrium of the Good Shepherd) and Confirmation classes do not take place at the same time as are K-6th program. 

Students currently enrolled and active, are promoted to the next grade each year and do not need to re-register. Catholic School students in grades 2, 7 and 8 MUST register.

In the event that my child's picture is taken during faith formation classes or related activities, I understand that there is a possibility that the picture may be displayed publicly on the church's website, in the local newspaper, parish social media sites, parish bulletin or bulletin boards etc.

A billing statement and information packet will be mailed to you in mid August. The current fee is $50 for the first child and $15 for each additional child, with a cap of $100 per family. Financial scholarship is available upon need.
