I, the parent or guardian of the above listed, release Townsend Bible Church from all medical and accidental liabilities in excess of the insurance coverage provided for these youth clubs. In the event of an emergency, you have my permission for emergency medical or surgical care in the event that you are unable to contact me. It is understood that any insurance we carry will cover any medical expenses first, before the church insurance is applied.

I understand that at this event or related activities my child may be videoed/photographed. I hereby authorize and consent to the editing, reproduction, exhibition, and use of said videos/photographs for use by Townsend Bible Church in its publications, on its website, or on social or local print media.

I represent and warrant that I am a parent or legal guardian of the child named above and have the full power and authority to enter into this Consent and Release of Liability on behalf of the child. By answering with my name below, I acknowledge that I have read and understood this document and also represent that all information provided is accurate.
