The purpose of this survey is to inform Free the Hounds about those greyhounds that have suffered a major on track injury or have suffered from or are suffering with cancer.

Free the Hounds may use information and statistics gathered from this survey in publications and other communications with the aim of improving greyhound welfare. Please complete a separate survey for each greyhound you have adopted with either of these conditions - living or deceased. Thank you for contributing to this research.
