In consideration of my participation in the sponsored activities of the ZLAX, I acknowledge, agree to and understand that:
- Waiver & Release of Liability:  I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses, associated with participation in a lacrosse event.  I further agree on behalf of myself my heirs, and personal representatives, that ZLAX, the host organization, and sponsors of this event, along with the coaches, volunteers, employees, agents, officers and directors of this organization, shall not be liable for any injury, loss of life or other loss or damage occurring as a result of my participation in the event, or as a result of equipment that may have been provided to me for these activities.
- Medical Attention: I hereby give my consent to ZLAX to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and/ or emergency medical services as warranted in the course of my participation in ZLAX summer clinics.
- READINESS TO COMPETE: I will only participate in ZLAX clinics/camps/games/tournaments for which I believe I am physically and psychologically prepared to compete.
- CODE OF CONDUCT:  I have read and agree to all terms in the ZLAX Code of Conduct, especially with regard to my responsibilities as a player.
-I understand that any ZLAX equipment borrowed must be returned at the end of the season in the condition in which it was loaned.  In the event that equipment is lost, broken or otherwise not returned the borrower will be charged the full replacement value of that equipment.
- I understand ZLAX is not responsible for injury that occurs while using loaned equipment.
ZLAX has my permission to use my or my child’s photograph or other media images such as videos publicly to promote ZLAX’s business interests. I understand that the images and videos may be used in print publications, online publications, presentations, websites, and for social media purposes. I also understand and agree that no royalty, fee or other compensation shall become payable to me by reason of such use.
