Thank you for accessing the Cabbage Patch Settlement House's Pre-Registration Form. By completing this form, you are taking the first step toward becoming a member of our community. Once you submit this form, a staff member will reach out to you to provide our full registration packet and schedule a tour.

Please keep the following in mind:

1. We ONLY accept new members between the ages of 8-16.
2. Please submit proof of your child's age either in person, or email (jlayfield@cabbagepatch.org) after completing this form.
3. Both you and your child will need to complete a tour and orientation before finalizing membership.
