Field Hockey Summer Clinic Registration Form
The 2021 CCHS Field Hockey Captains, assisted by the CCHSFH coaches, are offering a summer skills clinic to all players entering grades 6-9. The clinic will be held at the upper turf fields at Concord Carlisle High School.   8:30am-12:30pm on August 18th, 19th and 20th, 2021. The cost of the clinic is $150 with proceeds supporting the CCHS field hockey program as well as the coaches. In order to register, please fill out the form below.
We are cautiously optimistic that we can run a larger clinic than last year. If numbers are restricted, we will offer slots based on a first come basis, so please register early!

We will collect payment, health forms and waivers on the first day of the clinic. Payment is $150, and can be either cash, check to CCHSGFH. Health form, waiver and more information will be sent to registrants over the summer.
