This evaluation is to gain insight and evaluate how well you worked in a group and how well your group worked together. The first set of questions refers to your individual work in the group. The second set of questions refers to the group work effectiveness.
For all questions with a rating scale, 1 is low and 10 is high. Thus for question 1, you would rate yourself a 10 if you contributed your very best effort possible to the group presentation.
