We have received the maximum number of responses for our anticipated dog spay and neuter surgery availability through August, 2022 and are not receiving additional inquiries at this time. Please continue reading for additional information.

Submission of this form does not guarantee an appointment. If you submit your information through this form, you will be contacted to discuss scheduling.

Note: Due to high demand, we are only able to schedule a maximum of TWO (2) dogs per family per month. The ability to schedule two dogs per family on the same day, extra large dogs, cryptorchid male dogs or any other non-routine spay/neuter surgical request is dependent on availability and will be discussed when you are called.

Please note that once you are able to submit your information through this form and are then contacted to schedule, pre-payment for surgery will be required from all clients to hold your surgery spot.

Please do not plan on submitting this form for puppies or dogs that you do not wish to schedule for spay or neuter within the next two months. We are not able to schedule out farther than 2 months.
