What entities are included in your group? *
Required
Can you give a short description of the group above? *
(e.g., we are a family of 6, including an 85 year old grandma with diabetes, mom, dad and 3 kids aged 7, 10 and 12; I also want to manage my own routines and those of employees that work for me)
Current Challenges and Goals
*
(e.g., my household life is chaotic and unsustainable. I need more time to grow revenue in my business; I'm overwhelmed with caring for my mom and making sure she exercises 2x / day to manage her diabetes; my kids should be able to do their routines more independently;  my wife hates nagging everybody to do what they already promised they would do; I don't exercise and my blood pressure is too high; and I often have to to my employees' work because they aren't doing their jobs reliably and without supervision)
What routines might your group work on? (OK to start small and add later) *
(e.g., grandma: measure blood sugar, take insulin and walk 20m; kids: chores, homework and violin practice; myself: take blood pressure, exercise and focus on most important projects at work; employees: check customer inbox 2x / day, resolve issues and send a daily summary)
If you were to engage with MyA to get your routines in order, what would success look like at the end of one month? *
(e.g., grandma: reduction in blood sugar; kids: doing their routines 80% of the time; myself; reduced blood pressure and progress on revenue building; mom: more mental space to purge the home; employees: handle customers without supervision)
