TERMS AND CONDITIONS (Carefully read the following statement below before agreeing to them.) *

I understand that I will be scheduled on the basis of my counselor's availability.

I understand that if I do not show up for my session and fail to give notice prior to cancelling my appointment, I will have to approach CCC again in order to be rescheduled.

I understand if I fail to acknowledge the confirmation SMS sent to me one day ahead of my session, it will be automatically canceled and I will be rescheduled in accordance with the counselor's availability.

I understand that if I miss my session twice in a row without informing, my name will be dropped out from the counseling list.

I understand that the waiting window of my session is 15 minutes if I don't show up within 15 minutes of my session my counselor has the right to end the session.

By signing this I am agreeing to the above mentioned terms and conditions for counseling.
