Upon submission, you will receive a computer generated, email confirmation that your form has been received with all of your responses.Completion of this form does not guarantee enrollment. The TLC Program Director will notify you of your status.
The State of California requires updated registration for each child, so please submit a separate on-line application for each child enrolling in the TLC Program. Upon confirmation of an available space from the TLC Director, new families will need to submit a $50 Registration Fee to the ESUSD District Office at 641 Sheldon Street.
1) New families must complete one application per child and submit one $50 Family Application fee within *3 business days of confirmation from the director to maintain entry status.
2) Families currently enrolled must complete one application per child during the returning families registration period or be moved to the waitlist in timestamp order.
3) Families enrolled in prior years that are still on a waitlist must complete one application per child to update information and confirm ongoing interest in placement for the TLC program.
