調補課申請表－臺北市文山社區大學

本表單供講師、班級代表，申請 調補課、校外教學、線上教學 使用。填表前請注意：
  • 請提前至少 2 週申請。(春季班請於6/12前完成本期申請)
  • 請班代/老師協助通知同學：新的上課時間、地點等必要資訊。
  • 申請校外教學，請填寫集合地點、聯絡人姓名與電話。
  • 申請線上教學，請填寫線上教室網址連結，並請授課老師進入課務系統進行點名（參考操作說明）。

    春季班請於115/7/10前完成課程〈如遇天災無須補課〉

  • 春季班第 9 週（4/27 - 5/1）公民週講座活動，禁止補課。
  • 春季班第16週（6/12 - 6/18）原班團報週，禁止校外教學。
