Your UC-Berkeley email will be recorded when you submit this form but we will not share your name with the nominee unless you check "Yes" on the final question box.

Officers should only hold one Head (elected) position. Please fill out the form once for each person you are nominating.

This form will close on April 21. Elections will take place afterwards (4/22-5/5).
