Thank you for your interest in being a Founder and a Champion of the local, independent voice!  Please fill out this form to become a Founder and receive your benefits.  The donation required to be a Founder is $500, which can be made through our website or via check made out to Access Humboldt.  Once we receive the information below & your donation, you will be on the limited list of Founders for life and enjoy annual recognition, a Founder's pin, and the VIP experience at every BOLDtFEST Red Carpet World Premiere.  Because this opportunity is limited to 20 people, we are asking that one form per participant be filled out.  If you wish for a spouse/partner to participate, please fill out a separate form and make a second donation.
