This form is to be completed if you would like to sign up your child to be a participant in the Ballet and Books program. Filling out this form does not mean your child has been accepted to the program. Once you send in this form, you will be notified at least one week before the start of programming if we have space in this session for your child.

Your child’s participation in Ballet and Books is an excellent opportunity for extra literacy engagement support from a diverse group of students in addition to dance programming, free of charge. We hope this program will help your child gain confidence and a sense of accomplishment after working one on one with our student mentors and participating in our dance sessions. Because the program builds on itself each week, students may not continue with programming after more than two unexcused absences. Please email chapter leadership if you have any questions.

All our volunteers will complete mandatory training before being paired with a student. You are not required to be present during in-person instruction.

Any communications regarding questions about the program, scheduling mentor sessions, timeline of the program, etc., should be directed to your specific chapter by email.

Student mentors are instructed not to communicate with your child outside of the program sessions without you present. If you are concerned about the communication between your mentor and child, please immediately contact your local chapter by email.

For the safety of all participants, we may share your child’s first and last name with the location that programming is held at to assist with on-site attendance and emergency procedures. No other personal information will be shared.

Please read, complete, and submit this application in its entirety. You are not considered officially registered and enrolled until you receive an acceptance email from your local chapter stating that your child is in the program.
