NOTE: DIRECTORS SHOULD NOT COMBINE HIGH SCHOOL AND MIDDLE SCHOOL ON ONE REQUEST. IF BOTH PROGRAMS ARE REQUESTING A SITE CHANGE, THEY SHOULD SUBMIT THIS FORM TWICE.
