Teachers complete and submit this form as a request to transfer assessment events to a different site. Managers selected will receive an email, and will respond to verify they are aware of the circumstances.

NOTE: DIRECTORS SHOULD NOT COMBINE HIGH SCHOOL AND MIDDLE SCHOOL ON ONE REQUEST. IF BOTH PROGRAMS ARE REQUESTING A SITE CHANGE, THEY SHOULD SUBMIT THIS FORM TWICE.
