This is the application form of the project Creative Community.

We kindly ask you to fill all the questions in, so we can get to know you better. This programme is having 2 phases. An APV (advanced planning visit) and a Youth Exchange. Both they will take place in Ommen, the Netherlands.

Besides that, by answering these questions it will be possible for us to create the most welcoming programme for you!
