PHCPC Facilities Request
Thank you for filling out this request form. This form is required for all facility requests of Pleasant Hills Community Presbyterian Church.

BEFORE YOU MAKE YOUR REQUEST:
- Are you authorized, or have you received proper approval from a church elder or staff member? (Required)
- Have you allowed adequate time for the facility process? (At least 3 weeks in advance for significant requests)
- Are you ready to clearly articulate your request?
- Do you have all of the necessary details, including date, time, description, cost, and deadlines?

NOTES:
- We will work hard to effectively accomplish your request.
- Not all requests will be achieved exactly as desired or in the time desired.

A confirmation email will be sent to you within one week.
