Meeting Room Booking Request Form
This form is a request for a meeting space at MedFuse 2018.

○ Meeting times are in 30 minute blocks, and consecutive slots may be booked if necessary.
○ As soon as you book a room, it will not be available to other parties and the time slot will be removed from this form.
○ This is a request form, so these requests do not guarantee booking. However, we will do our best to accomodate everyone.
○ A minimum of two parties are required to book a meeting room, and both parties must accept the invitation to a calendar invite via the emails submitted in this form for the room to be fully booked.
○ Please only submit a request with the full-intent of using that space.
