InnoWorks 2019 Online Application
Welcome to the 2019 InnoWorks Workshop Student Application!

Since 2003, InnoWorks has successfully conducted 50+ summer workshops serving over 2,000 InnoWorkers (InnoWorks students), benefiting from the contributions of over 1,000 volunteers.

IMPORTANT: Please complete the application if you are interested in being considered for participation in the 2019 InnoWorks Program. Please read the instructions below very carefully and follow the directions exactly so as not to lose your work. Be sure to complete all parts of the application and send in the signed application in order to be considered. If you have any questions, please contact applications@innoworks.org.

Application Directions:

1. Please fill out each page completely before going to the next section. ONLY pages that require a signature need to be printed (as noted in the next step).

2. Pages that require a signature MUST be printed. Please follow the instructions to print ONLY when prompted before moving onto the next section. Please note that ONE signature is required from the student while the others are required from parent(s) / legal guardian(s). If desired, you may print out each page as you complete them for your records.

3. Send the signature pages, copies of the front/back of the health insurance card (if applicable), and report card via the United States Post Office (USPS) (other carriers may not deliver to PO Boxes) to:

If it is more convenient, the health insurance card and report cards can be scanned and emailed to applications@innoworks.org instead of mailed. However, pages with the ORIGINAL signatures MUST be mailed to the mailing address above.

NOTE: InnoWorks has very limited resources, so when all conditions are equal, the earlier completed application as determined by the submission time stamp and arrival of signed paper application and supporting materials will be considered first.
