Bay Area health officers, in alignment with CDPH, continue to strongly recommend masks be used as an effective tool to prevent the spread of the virus especially when case rates are high, or when additional personal protection is needed. Continuing to mask in indoor public settings, especially crowded or poorly ventilated spaces, remains the safest choice for an individual and protects those who are medically vulnerable or are not able to get vaccinated, like our youngest children. Although the County of San Mateo lifted its universal mandatory indoor mask requirement on Wednesday, February 16, the MEF Gala COVID-19 Committee will implement the following requirements for entry into the Gala:

All participants are highly encouraged to wear masks indoors.

Only fully vaccinated individuals (including a booster shot) will be allowed entry into the Gala premises.  
 
Please present your vaccination card and valid, government issued identification card at the event entrance.

By providing my full legal name below, I am agreeing to the MEF Gala 2022 requirements and will adhere to the protocols set forth by the Gala Covid-19 Committee.  If I do not provide a valid proof of vaccination AND a valid, government-issued ID card, I understand that I will NOT be able to enter the Gala premises.  If I am feeling ill or have been exposed to Covid-19, I will not plan on attending the event.
