Holmes Attendance Form
This form is to be completed by PARENT / LEGAL GUARDIAN to report FULL DAY absences or LATE ARRIVALS for Holmes students.
  • In order to be excused, an absence must be reported by NOON the DAY FOLLOWING the absence.
  • Absences may be reported in advance.
  • A confirmation of your submission will be automatically sent to the email address you provided.
IMPORTANT!  If your student needs to be dismissed early from school, DO NOT USE THIS FORM!  You will need to COME TO SCHOOL WITH YOUR ID to pick them up.  (Students will only be released to contacts who are listed on their Emergency Card.)
