Special Children & Youth Ministry Programs Registration 2018-2019
Note: These programs are only open to children and youth whose families are pledging members and associates of the congregation in good standing (with an up-to-date pledge). These programs fill up fast and require a significant commitment from volunteers and staff so they are reserved. This form should be filled out for EACH child in your family who will be participating in Special Programs for Children and Youth Program in 2018-2019. Special Needs
Does your child or youth have a special need that we should know about? Learning disability, physical disability, autism, ADHD, allergies, fears, illness, or other challenges. Please explain briefly. This information will be shared with staff and volunteers working with your child. Also indicate if you would a staff member to contact you.
