This form is to be used by all building staff in the event that you have any technology issues with regards to repairs and trouble shooting problems. This may include issues with hardware, software, projector bulb replacement, printer problems, student laptops, teacher laptops, desktops, chrome books, iPads, Windows 10, damage to technology, and questions you may have.
Your name or the name of the person needing assistance.
Location of concern and room number. (Example- room 127, document camera located on teacher's desk).
What are you needing help with?
Optional- use this space to provide more detail to your concern and provide a description of any troubleshooting you might have done.
Make or Model of Machine ( Ex. laptop 5530 or Desktop 380).
Service tag or Serial Number of Machine. (Do you best to find this on the bottom, or back, of the computer if you can- It should contain both numbers and letters).
