VOE Request Form
NOTE THAT VOE REQUESTS HAVE A 3-5 DAY PROCESSING TIME.

This is something you NEED for getting your learner's permit or driver's license. It is only good for 30 days.

Note that requesting a VOE does not guarantee that your student will get one; students must be passing their classes and have an average daily attendance of 90%. If this is not the case, they will need to talk to their AP. 

Parents: if you are filling out this form for your student, please enter THEIR email. I will be sending an email to them letting them know if it's ready to be picked up/if they need to see an AP. 

Students: please write down the email that you check more often; it does not have to be your AISD email.
