2016-2017 Device Replacement Program Sign Up Form

Dear Parents and Guardians,

As we move into the digital age, we are focusing our resources on providing Chromebooks for all of our students. These devices are critical tools that the students will use to demonstrate their learning in ways that were not even possible a few years ago. With the device comes responsibility for its safekeeping. We also know that accidents happen and because of that, we are offering an optional Chromebook Replacement Program.

The program costs $30 per student. This is an optional, annual program that is open to all students. In the event your child’s chromebook needs to be replaced, you will have the option of joining the program for a second time at a cost of an additional $30. If you are not a part of the program and your child’s chromebook is damaged, you will be required to pay the total cost for repairs or replacement (up to $200 per event).

In order to participate in the Chromebook Replacement Program, please complete the form below and send the $30 payment to your child’s school secretary. Or, you may follow the link at the end of the survey to use the e-pay option through the District 200 website. In the “Type of Miscellaneous Payment” box, please enter “Chromebook Replacement Program". You will also need to manually enter the cost of the program, $30, in the “Miscellaneous Amount” section.
