I understand that students participating in this club will receive a Garmin Vivofit electronic device to wear throughout the eight-week program.  I understand that this device must be returned to CHS following the program.  If the device is not returned, I understand that my student and I will be held liable for replacement cost, not to exceed $47.  My student will be responsible for syncing his/her device and reporting step totals to the club sponsors.

I understand that students involved in the Fitness Club will be training together to walk the 5K or 1-Mile Center Education Foundation event on October 1st at 8:00 a.m.  This event is FREE of charge for all Center students and we will be walking together as a club.  

My initials below indicate that my student has my permission to participate in the Center Education Foundation/Fit 58 5K or 1-Mile Walk.  I know that walking or running a 5K or 1-Mile event can be a hazardous activity.  I understand that my child should not participate if medically unable to do so.  I assume all risks associated with my child's participation. Having read this waiver and knowing these facts and in consideration of your accepting my entry fee, I, for myself, and anyone entitled to act on my behalf, waive and release the Fit 58 Fitness Club sponsors, Center School District, Center Education Foundation, City of Kansas City, all sponsors, their representatives and successors, for all claims or liabilities of any kind arising out of my participation in this event.
