Emergency Medical

As the parent or legal guardian of Junior, I hereby give consent to the Club to obtain all emergency medical or dental care necessary, at the Club's discretion, to preserve the life, limb, and/or well-being of Junior. I acknowledge that the Club is not a healthcare provider, and my provision of the information herein will not be construed to create any duties on the part of the Club.

Liability/Medical Release
Junior and I do hereby fully and forever release, acquit, and discharge The Hills of Lakeway (“Club”), ClubCorp USA, Inc., their respective current and former principals, officers, directors, managers, employees, agents, parent companies, subsidiaries, affiliates, and representatives of any kind from and against any and all claims, demands and causes of action of every kind of nature, for any and all damages, in relation to the Club’s sponsoring a junior tennis program and all junior tennis-related programs and my Junior's participation therein. Junior and I voluntarily and freely choose to assume all risks and dangers that may be associated with, or result from, the participation of the activities described herein.

Photo Release

As the parent or legal guardian of Junior, I hereby give my consent to the Club to use photographs of my Junior for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.
Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

Weather Policy

Due to our continuous concerns for our Juniors' safety and the safety of their families. The World of Tennis reserves the right to cancel, without notice, any Junior classes during inclement weather (rain, storm, threat of thunderstorm, cold weather, etc.). We recommend calling our Tennis Pro Shop (512) 261-7222 before driving in during days of uncertain weather.

Non-Member Policy

A non-member is welcome to attend the program provided there is space available for a member. The club reserves the right to cancel enrollment of non-member if space in class is needed for member of The Hills CC. Non-member fee is additional $15% of class fee.

Withdrawal and Change

All Changes to an existing program registration are required to be provided to the Program Director in writing 30 days PRIOR to the registered session.
In the event a Change occurs, for any reason LESS THAN 30 days prior, the Club will retain 100% of the session fee.
