Counseling at Seabrook Elementary School

Hello!

This is the Seabrook Elementary School Counseling referral form for the 2025-2026 school year. If you have any mental health concerns regarding a student, please complete all sections of the form to the best of your knowledge.

The counseling office at SES can provide:
* Individual Counseling (typically 6-8 sessions)
* Group Counseling (within the same classroom/grade-level cohort)
* Consultation and referral for outside counseling (either in-house Seacoast Mental Health Center or outside provider)
* As needed/drop-in/check-in service

Parents will be asked to sign a permission for counseling before any school counseling services begin (exception: in times of crisis, emergency, or safety situation).
