Replacement or Never Received Certificate of Completion
Form is provided for individuals that have not received or have misplaced a certificate of completion for associated courses through the Public Safety Training Center.
Any course recently taking, may not be processed up to 30-days post completion, and thus only use this form, if you are sure you never received your certificate of completion. We also recommend you check your spam, prior to completing this form.
Note: Spring of 2020 all certs will be electronic, and will be received by email. If you did not receive a cert in a timely manner, 30-days from course completion - it is possible we had either no email, or a bad email for you.
Upon completing this form, you will receive a confirmation of the form being completed via email. If you did not receive one, then you either provided the wrong email or it is in your spam folder.
We will attempt to complete your request within one (1) business week of receiving request.
