This Google form will collect all records pertaining to Citizenship /SR Card services in a common sheet, where all staff can have access to it. This record will be used for various purposes like generating of revenue report or any sort of reports in regard to CID/SR card applications.
The information entered below will be reflected in CID Processing part & the CID Issuance part is where we enter in Google Sheet manually.
If you make double entry and delete from the backend, the data in processing part will move one row above while CID Issuance card will remain unchanged. PLEASE AVOID double entry to have accurate information.
