ASA Sports Registration for C-I Sports
Please fill out this form to begin registering your student for middle school sports. Please note that the options listed are provided for 7-8 graders. 

Bussing: ASA students may ride a CI bus to either Cambridge Middle School or Isanti Middle School. If your student is already using the bus, you are set. If your child is NOT a student who rides the bus, please fill out this form.

Please pay the registration fee here.**

The MSHL Physical Examination form and MSHL Eligibility Form need to be completed and submitted to the ASA Middle School Office in order for your student to participate in C-I sports. 

Please send both completed forms to:  enrollment@asa.k12.mn.us or drop them off in the MS Office. 

Please Note:
** If your family is approved for FREE/REDUCED school meals through the completion/submission of the Application for Educational Benefits form for the current school year OR through Direct Certification based on participation in:   Minnesota Family Investment Program (MFIP), Supplemental Nutrition Assistance Program (SNAP), Medicaid, or Food Distribution Program on Indian Reservations (FDPIR) - they may be eligible for reduced athletic participation fees. 

If you feel your student is eligible for reduced athletic participation sports fees, please call Ms. Susan 
(MS Office Mgr.) at: 763-444-0039 to confirm your students eligibility BEFORE paying the registration fee.

Once your student is registered, the fee is paid, and the Eligibility form and Sports Physical form are turned in, they are all set!
