Amaze Feedback Advisor Signup Form
Thank you for interest in becoming a Feedback Advisor.

As you may know AMAZE (a partnership between Advocates for Youth, Answer and Youth Tech Health) is working to create an online space for tweens and young adolescents to learn about puberty, body image, healthy relationships and sexual health.

We’re building out a Feedback Board of young people, parents, and educators! We’re looking for people to provide feedback on our videos and materials as they are being developed. We want to ensure AMAZE is a useful resource for parents and educators – whether it be used to help start conversations at home, or to supplement lesson plans in
school. We’ll be asking for feedback through occasional conference calls, focus groups, and surveys.

The goal of the project is to produce animated videos to provide honest and engaging information to normalize sexual development and help young adolescents (ages 10-14) develop health promoting attitudes, knowledge and behaviors they need to navigate the critical transition between childhood and older adolescence. We will strive to gain the interest and the trust of very young adolescents (VYA) in order to counter the misinformation that is so pervasive. These short edgy videos and other materials will provide accurate information about puberty, body image, healthy relationships and sexual health that is both age-appropriate and medically-accurate.

All survey participants will receive rewards each time they participate in a survey. Youth will receive a $15 signup reward (Amazon gift card) once they have signed up, we have received their parent's consent and we have selected them as an advisor. Parent consent forms will be sent after you have signed up.
