Home Learning 'Track & Trace' Monitoring Form
Please complete this form if you are home learning and your child develops either COVID 19 symptoms, books a test or receives a positive COVID-19 test result.
The DfE have asked schools to inform them of numbers of cases in the community in line with the increased expectations on schools to perform track and trace duties.
I would like to reassure you that no personal information is shared with the DfE about COVID-19 cases, just numerical data as we have to report to the DfE daily on all the children on our school roll.
