Transcript Request Form for Current Students
-University of California (UC) and the California State University (CSU) systems do NOT require transcripts to accompany the application. Once a student is admitted to a UC or CSU and commits to attending that university, the student MUST send a final official transcript in June.
-Transcripts will be sent via Naviance to Universities applied on Common App.
-SAT and AP scores must be sent directly from the College Board.

Official transcripts will be picked up in person and your signature will be required. It is your responsibility to mail out official transcripts to your designated university or program.
