Meal Request Form
Easton Valley Schools will again be offering our meal program, for families needing assistance during the school closure, starting on Monday, April 20, 2020. MEALS WILL BE FREE TO ALL STUDENTS AGES 1-18. Please complete this form by 8pm the evening prior to the day you would like a meal so we can accurately prepare the meals. School staff will be preparing the meals and CURB SIDE pick up will be utilized for all locations and meals will be available for pick up on MONDAY, WEDNESDAY AND FRIDAY. We will be delivering two meals on Monday and Wednesday and one on Friday to reduce the potential exposure to any staff or students.
Pick up times are listed below:
- Preston 11:30-12:00
- Miles - 11:30-12:00
- Sabula - 11:30-12:00
- Spragueville - You will be contacted for a drop off time
- Green Island - You will be contacted for a drop off time.
When completing the form, please indicate which town location you will be using to pick up your meal(s).
