Club Polices
Crescent Tennis Club has polices (and guidance) on welfare/safeguarding, diversity and inclusion and data protection. 

We recommend that all parents and carers know who our welfare officer(s) are and how to contact them (details are on our website and in the clubhouse)

We also have other polices together with details of our terms and conditions.

We would ask that parent make themselves familiar with these and speak to their child about our expectations.

Photography and filming
From time to time will may wish to use child your image for promotional use, including on the Club's website, social media or in our newsletter.

To do this we need to seek your agreement to release any images taken and consent to use those images.
You do not have to agree and consent can be withdrawn at anytime by informing the Club.

Juniors Playing Adults (including transportation to other venues)

It is the Club’s policy that only junior members aged 13 -18 and on the recommendation of the clubs captains or lead coach may play  adults at the club or representing the club in league matches or other events, including the club annual championship.

We require that parents/carers of ALL juniors to be responsible for transporting their children to or from tennis venues and events and maintaining a presence when there to supervise their child.

Please note we do not make direct contact with children - therefore we are asking you about how you would like to b contacted. by the Club. You may select all, some or none of the following;
Medical Treatment - Health Declaration
If it is necessary for my child -( if I am unable to be contacted by my emergency contact details or other means) to require emergency medical treatment, I hereby give my general consent to any medical treatment and authorise the Club representative to sign any document required by hospital authorities.

We are required by the LTA to ask is your child  are fit to play and is not doing so against a GP or hospital advice.

On completion and receipt of this form - our Membership Secretary will contact you if your application is successful and inform you of our payments methods and how to pay.

If you are renewing your membership you are required to complete this form and once submitted you will receive details on how to make your renewal payment.

All fees are for the year (or if applicable pro rota) and are non refundable - please see our terms and conditions which can be found on our website.
