Schoology Parent Portal Access Form
Please out one (1) form for each student in which you are requesting access to Parent Portal. If you have multiple children in grades 6-12 you will need to submit multiple forms.

This form can also be used to amend an e-mail address for any contact with Parent Portal access as this information will affect log-in status.

Please note:
- Schoology is only used for students in grades 6-12.
- Parent Portal access will only be given to those listed as the student's parent or legal guardian. If you need to update this information, please contact the main office of your child's school.
- Once processed, Schoology login information will be sent to the email address you provided in this form directly from Schoology.
- Please allow 3-5 school days to receive login information as your request for access needs to be verified. If 5 school days have passed and you have not received an email from Schoology, please contact the building secretary.
