Please complete this form to request a change to your child's current transportation arrangement.
A separate form must be completed for each child that you are requesting.
Only the following two change request reasons will be honored by way of this form:
1.) Termination of Service, or 2.) Change of Bus Stop.

Change requests having to do with Hardship and Safety concerns should be sent directly to jkaiz@d57.org.
Change requests having to do with IEP Accommodations should be sent directly to styburski@d57.org.
