Tutoring Request Form (Returning Students)
Thank you for taking the time to fill out this form for returning students.

Recent changes:
1. Salinas Tutoring is now KAP Tutoring Network, which currently consists of 3 tutors: myself (Alicia Salinas), Dominique Sharp, and Sarah Stewart. If you'd like, you can learn more about them under the "About Us" tab at kaptutoring.com.
2. All online and in-person tutoring meetings are scheduled for either 50 or 80 minutes.
3. All parents/guardians will receive a link to a google drive folder containing lesson outlines, which will keep you up to date on what topics we worked on during our meetings and any notes or suggestions for further practice.
4. In addition to Venmo, we can now accept these forms of payment: PayPal, Credit, and Debit. Venmo and PayPal requests may be sent up to a week ahead of scheduled meetings, but the payment deadline will remain the same. If you choose to pay using credit or debit, you can also request automatic payments for meetings.

CANCELLATIONS, PAYMENT, AND TECHNOLOGY POLICIES
Payment Policy:
Tutoring payments are due and payable 24 hours before the scheduled tutoring session for the first lesson with a new student and 24 hours after the scheduled lesson for returning students. If you miss the payment deadline, you will be charged an extra late fee in addition to the cost of the lesson. This late fee will be calculated at 10% of the cost of the lesson, and any future scheduled meetings will be put on hold until payment is received.*

Cancellation Policy:
The cancellation deadline is 24 hours before a scheduled meeting. If you choose to cancel your meeting after payment and before the meeting cancellation deadline, you will be fully refunded. After that cancellation deadline you will be charged for the full cost of the cancelled lesson.*

Technology Policy:
It is the responsibility of the student and/or their guardian to check that the technology devices and internet connection they are using are in working order before the start of the meeting to ensure uninterrupted meetings. Please follow proper technological care procedures and reach out to IT professionals whenever issues arise.

*Any exceptions to these policies are at the discretion of the tutor.
