Behavioral Health Provider Complaint Form

The Colorado Division of Insurance (DOI) accepts complaints from mental health and substance use disorder (SUD) providers, also called "behavioral health" providers, regarding issues with commercial health insurance companies in Colorado. Information received from providers helps the Division to document issues, identify trends, and informs the DOI's Mental Health Parity and Addiction Equity Act (MHPAEA) enforcement actions in order to protect Colorado consumers.

PLEASE NOTE:  The DOI does not regulate Medicaid, Medicare, Medicare Advantage, CHP+, SHIP, VA-administered, or self-funded plans, or any health plan issued outside of Colorado.

The DOI is statutorily prohibited from getting involved in provider-carrier contract disputes. Providers should use the dispute resolution process outlined in Colorado Insurance Regulation 4-2-23 Procedure for provider-carrier dispute resolution. Further, providers should encourage patients (the policyholders) to file complaints on their own behalf through the consumer complaint portal on the Division's website.

You will be able to use this form to provide information regarding issues with one company. You are welcome to submit more than one form for additional companies.
