In order for your financial planner to structure a recommendation it is important that you take time to consider whether the information that you provide is as accurate as possible as the information provided forms the basis of the advice given to you. By completing this form you confirm that the information provided is accurate to the best of your knowledge. In the case of married or partnered individuals it is crucial to provide information on both parties as part of this review - even if only one party is being reviewed / discussed / attended the meeting.

Before completing a financial review or providing any financial advice to our clients we at MyWealthManagement feel that its important to determine that there are no special circumstances which could impact the advice we give you. We would ask you to take a moment before progressing to consider any personal circumstances that you feel we should be made aware of that could potentially impact our recommendations.
